Mental Health Targeted Case Management (MHTCM)

At ThriveWell Counseling Solutions LLC, we provide Mental Health Targeted Case Management (MHTCM) to support individuals who require structured help in coordinating care and services. This offering is part of our outpatient behavioral health services available in Chicago, Illinois. It is intended for those diagnosed with persistent mental health conditions and who need ongoing assistance navigating health, behavioral, educational, and related systems. Clients are identified through a standard assessment process to determine if this level of support is appropriate. The purpose of MHTCM is to assist individuals with staying connected to essential resources that impact their daily responsibilities and wellness.

Eligibility

MHTCM may be available to individuals who:

• Are any age
• Have a diagnosis of one or more chronic mental health conditions as outlined in the current Diagnostic and Statistical Manual of Mental Disorders (DSM)
• Are determined through uniform assessment to require targeted case management services

Purpose

This service helps connect individuals to:

• Health-Related Services
• Social and Behavioral Support Systems
• Educational Resources
• Other Coordinated Assistance as Needed

Types of Case Management

• Intensive Case Management

  Support is delivered mainly in community-based settings, with services provided directly to the individual or to their legally authorized representative (LAR). The focus is on coordinating and facilitating access to care that addresses complex needs. The client may or may not be present during interactions. This type of case management typically involves frequent contact and higher levels of engagement.

• Routine Case Management

  These services are mostly provided in-office and assist the client, caregiver, or LAR in managing care needs. Routine case management is structured to provide consistent access to the services necessary for ongoing support.
