ThriveWell Counseling Solutions LLC offers Mental Health Rehabilitative Services to help individuals manage long-term conditions that interfere with everyday functioning. These services are delivered as part of our outpatient behavioral health program in Chicago, Illinois. Support may include working on behavioral regulation, improving social participation, and managing psychiatric symptoms in a way that promotes community involvement. Individuals must meet clinical criteria and have a need for specific interventions outlined in a service plan. Each component is designed to reduce disruptions caused by mental health symptoms and to enhance day-to-day engagement.
Eligibility
Services are available to individuals who:
- Are diagnosed with a serious mental health condition
- Meet diagnostic standards listed in the current DSM

- Community interaction
- Sustaining residence in the community
- Behaviors associated with serious mental illness (SMI) or severe emotional disturbance (SED) that limit participation in everyday routines

- Medication Management
Medication-related support, including evaluations, prescribing, monitoring, and instruction for safe and effective use.
- Psychosocial Rehabilitation
Training to help improve daily routines, social interaction, and communication for use at home or in group settings.
- Skills Training & Development
Guided activities are designed to assist individuals in building or strengthening personal, home-based, or community-related tasks.
- Crisis Intervention
Short-term behavioral support for individuals experiencing psychiatric or emotional distress requiring immediate attention.
